To receive a payment under this subchapter, a peace officer must provide to the attorney general: (1) proof that the injury: (A) was sustained in the performance of the applicant's duties as a peace officer; and (B) is a total disability resulting in permanent incapacity for work; and (2) any other information or evidence the attorney general requires.
