If an applicant for a protective order is a former spouse of the individual alleged to have committed family violence, the application must include: (1) a copy of the decree dissolving the marriage; or (2) a statement that the decree is unavailable to the applicant and that a copy of the decree will be filed with the court before the hearing on the application.
